If you are interested in playing an intercollegiate sport at Wentworth this year, there are several forms you need to fill out before you may begin practicing. This year all of the forms are being done electronically. You have direct access to the institutional forms through the Current Student-Athlete section of the Athletics website, but in order to access the NCAA-mandated forms, you need to fill this short form out so that you can be activated in the NCAA system. Once you have been activated, you will receive an email with a link to the NCAA forms. This email will not be automatically generated and may take a couple of days to be processed, so please do not fill out the form more than one time . Once you have submitted this form, the confirmation screen your form has been recorded in our system for further processing. Please note that you can not fill out the NCAA forms until you have been registered in the system as each student-athlete is assigned a different code and the code changes from year to year. Forwarding the email to anyone who hasn’t been registered in the system will not work for those recipients. Please be advised that any forms received during the timeframe of July 12-July 23 will not be processed until July 24, at the earliest.

Please note that prior to being able to try out for any intercollegiate team the following items need to be completed:

Athletic Training Paperwork (online forms)
NCAA Paperwork
Student-Athlete Biographical Form

All paperwork - both compliance and medical - must be completed prior to August 1 in order to be allowed to try out for a varsity athletics team. Please note that compliance and medical paperwork are two separate entities.
