Compliance Forms Now Available

UPDATE (07/29/14 - 10:00 AM):  The NCAA Compliance Forms Portal is back up and running. All forms submitted prior to the afternoon of July 28 have been pushed through the NCAA portal and emails have been sent out. Thank you for your patience!

Dear Wentworth Students,

This email is for ANY student – both returning students and incoming freshmen/transfers – who has an interest in trying out for any varsity athletics team during the 2014-2015 academic year.

If you are interested in playing ANY intercollegiate, varsity sport at Wentworth this year, there are several forms you need to fill out before you may begin practicing. You now have direct access to ALL of the institutional forms through the Current Student-Athlete section of the Athletics website (everything is being done electronically, except for the pre-participation physical form, which can be downloaded as a PDF), but in order to access the NCAA-mandated forms, you need to fill a sport registration form out so that you can be activated in the NCAA system. Once you have been activated, you will receive an email with a link to the NCAA forms. Registration forms are typically processed through our system and uploaded to the NCAA site within one to two business days. Please be advised that no forms will be processed from July 17-28.

Please note that due to a reconfiguration of the NCAA site, ALL student-athletes (new and returning) need to fill out the Sport Registration Form. We apologize for the inconvenience.

Once you fill out the registration form you will have received an email from the aforementioned “nobody(at)ncaa(dot)org,” which contains your login information. Again, this process and all of the other forms under this section of the website are mandated for anyone who wishes to try out for a team. Just because you were a member of a team last year does not exempt from doing this.

Please note that you can not fill out the NCAA forms until you have been registered in the system as each student-athlete is assigned a different code. Forwarding the email to anyone who hasn’t been registered in the system will not work for those recipients. Additionally, you have seven (7) days from the time the email is sent out in order to complete these forms before the system times you out and you will need to be reset through our office. If you are under the age of 18 when you fill out the registration form, you need to also fill out the PDF version of the NCAA Drug Testing Consent Form, as it requires a parent or guardian signature. If you are over 18 when you’ve registered with the NCAA, please do not fill this form out.

In order to streamline the process there is only one internal form that needs to be filled out online, the 2014-2015 Student-Athlete Biographical Form. This form combines the historical questionnaire and the sports information questionnaire. With this new process, all student-athletes are required to fill out this form each year, however returning student-athletes are required to fill out a much shorter version. NCAA forms will always be required of all student-athletes every year as that is an NCAA rule, not a Wentworth rule.

When filling out the Student-Athlete Biographical Form, you will know you have done them correctly provided:  once you hit “Submit,” you will be taken to a review screen where you will have the chance to confirm all of your responses and in the event there are any questions that have not been completed, they will show up in red and you will need to go back (by clicking “re-edit form”). If everything is correct, please click the “Please Confirm” button. This will bring you to a confirmation screen that says:  “Thank you for submitting your form.” If this screen did not show up, your form was not received. Instructions are attached to this email.

Regarding the medical forms, which came under a separate cover from the Athletic Training Office, it is strongly suggested that students get on top of this during the summer months to alleviate any problems there may be prior to the start of your season.
